Care Manager

A care manager is a healthcare provider who assesses patients’ needs and creates personalized care plans. At times care managers act as supervisors at facilities such as nursing homes where they manage business operations and patient care. Also known as a patient care manager, care coordinator, or patient care coordinator.

Job Descriptions

We are looking for a detail-oriented service manager to manage our service department and handle customer service interactions, reports, and repairs. The service manager’s responsibilities include providing the highest level of customer service, developing lasting customer relationships, and managing the service team members. The service manager maintains a strong working knowledge of all industry standards and practices as well as the company’s products and services.

To be successful as a service manager, you should display a strong sales-minded attitude as well as excellent leadership skills. You should also have the ability to develop and maintain strong, positive relationships with customers and third-party vendors.

Job Responsibilities

  • Creating and updating personalized care plans for patients
  • Collaborating with patients’ families, friends, and social supports in developing treatment plans
  • Identifying and recruiting high-quality healthcare providers
  • Interacting with healthcare providers to facilitate patients’ treatments
  • Ensuring patients’ treatment requirements are met
  • Suggesting alternative treatment plans when patients’ service requests do not meet medical necessity criteria
  • Instructing and educating patients on procedures, healthcare provider instructions, and referrals
  • Linking patients to social services programs and entitlements

Job Requirements

  • Bachelor’s degree in social sciences, social work, nursing, or a related field
  • 2+ years of work experience in public health or related field
  • 1+ years of current case management experience
  • Registered Nurse (RN) license
  • Certified case manager (CCM) license preferred
  • Proficient with Healthcare Management Systems and Microsoft Office Word and Excel
  • Outstanding communication skills, both written and verbal
  • Excellent organizational skills
  • Ability to travel locally
